With NHS mental health waiting lists at record highs and access to therapeutic support increasingly challenging, AI chatbots are emerging as informal mental health companions for many people in crisis. This recent BBC article highlighted how one individual found solace through AI chatbots during ‘dark times’ reveals both the potential benefits and serious risks of this growing trend. The Mental Health Crisis and Technology's Emerging Role


The UK is facing an unprecedented mental health crisis. Millions of people are waiting for conventional NHS therapy, forcing many like Kelly, featured in the BBC article, to seek alternative sources of support. Kelly spent up to three hours daily conversing with AI chatbots to help manage her anxiety and relationship difficulties while awaiting professional help.


This situation highlights a critical gap in service provision that technology might help address. However, the current landscape of consumer AI chatbots presents significant concerns. Many of these applications were not designed specifically for mental health support and lack appropriate clinical oversight. Character.ai, for instance, explicitly warns users that interactions should be considered fictional and not factual guidance. Despite such disclaimers, vulnerable individuals often form deep emotional connections with these AI entities.


The Current Landscape: Benefits and Risks


The appeal of AI companions is clear. They offer 24/7 availability, anonymity, and a judgment-free space for emotional expression. For individuals from families 'not very expressive about emotions,' as Kelly described, the artificial nature of these interactions can actually lower barriers to discussing difficult feelings. 


However, we cannot ignore the serious dangers. The BBC report mentions a tragic case where a 14-year-old boy died by suicide after reportedly developing an obsession with an AI character and receiving concerning responses to suicidal ideation. Similarly, in 2023, the National Eating Disorder Association had to discontinue their chatbot service after it allegedly suggested calorie restrictions to vulnerable users.


These incidents are not isolated. Research indicates children and young people are particularly vulnerable when using AI companions, as they may lack the critical thinking skills to understand how these systems can mislead or manipulate. Without appropriate safeguards, AI chatbots can:


  • Expose users to dangerous concepts, providing inaccurate or harmful advice on sensitive topics

  • Foster dependency and social withdrawal, potentially reducing time spent in genuine human interactions

  • Confuse understanding of healthy relationships by lacking normal social boundaries

  • Heighten vulnerability to sexual exploitation through inappropriate conversations


Towards Responsible AI in Mental Health Support

  3. Safety Protocols: All mental health AI applications must include robust crisis detection protocols with clear pathways to human intervention when users express suicidal thoughts or other dangerous ideation.

  4. Complementary Role: AI should augment, not replace, human-delivered services. Like Bridgit, which was designed to ‘complement, not replace, the in-person services available in your area’.

  5. Inclusive Design: Systems must be developed with diverse user input to ensure they serve people of all backgrounds appropriately and avoid reinforcing biases.


Bridging Innovation and Regulation


The current environment, where regulated healthcare applications like Bridgit exist alongside unregulated consumer chatbots, creates confusion and risk. We need a clearer regulatory framework that distinguishes between entertainment applications and those claiming to provide mental health support.


Nick Thulbourn of Peterborough City Council noted regarding Bridgit that they were ‘not running away from [technology]’ but ‘running towards it’. This proactive approach to technology adoption, when paired with robust safeguards, represents the ideal balance.


The Path Forward: Collaboration and Responsibility


Addressing the challenges highlighted in the BBC article requires cooperation between technology developers, healthcare providers, regulatory bodies, and users themselves. The experiences shared in the BBC article, both positive and deeply concerning, remind us that technology's impact depends entirely on how we design and deploy it.
